When making your initial physiotherapy booking, it is important to let us know if your intention is to claim Physiotherapy through Workers Compensation.
Payment at the time of your consultation is required, until all documentation has been completed and approved by your insurance company.
For your Physiotherapy Appointment:
Please download a Workers Compensation Form, and bring the completed form in with you for your initial consultation. We also have these forms available at reception, if you’d prefer to complete it on the day.
Workers Compensation Claim Form
To fill in your form, you will need the following information:
- Claim Number
- Insurer Details including case manager’s name, phone number, fax number
- Employer Details
- Medical Certificate
- Date of Injury/Accident
This information will permit your treating Physiotherapist to submit the necessary paperwork to your insurer, regarding the treatment and management of your specific condition.
